An illustrated clinical update on necrobiosis lipoidica

類脂質漸進性壞死−圖文並茂的臨床更新

Abstract

Necrobiosis lipoidica (NL) is an uncommon granulomatous inflammatory skin problem, which can be associated with diabetes mellitus. NL is often chronic and the lesions typically affect the lower limbs. NL may result in distress and disfigurement, especially in young women. The pathophysiology of NL is currently not well understood. Local and topical treatments remain the mainstay treatment. The ongoing management of recalcitrant NL, especially ulcerating disease remains challenging. Treating physicians of advanced cases need to be aware of the significant psychosocial impacts of this debilitating and sometimes painful skin disease.

類脂質漸進性壞死是一種罕見的皮膚肉芽腫炎症性問題,多為慢性的,病灶常見於下肢,可以和糖尿病有關聯。此病影響身心,尤其是在年輕女性患者,可因容顏受損而導致心靈創傷。目前,類脂質漸進性壞死的病理生理學還未清晰,其治療仍是以局部和外敷的為主。對於頑固個案的治療,特別是潰瘍性病例,仍是一大挑戰。主治嚴重病例的醫生須注意到,這可以是一種非常難纏並時有痛楚的皮膚病,對患者的心理及社交俱有著極大的影響。
